Pen and notebook is what we have done. The kids get lists from individual teachers/classes the 1st days of school and have a few days to get it all in order.

I recommend buying some stuff ahead of time. After school has started the school supply section at Target is really sparse and the line at Staples is insane. I stock up on college ruled paper, pencils, highlighters, pens, and folders during August to make sure I can actually get them. Can't go wrong with a couple of 3 subject notebooks and a composition book or two either. When school starts I have my kids first "shop" from my pile of supplies and then I take them out to choose whatever is remaining.
Anything extra stays in our "homework drawer" for future use, but I really don't think we've had much (if any) waste.
I wouldn't buy binders because they are expensive and teachers can be very specific about 1" , 2", 3" etc.
